If you are new, please be sure to fill out the following forms, and bring them to your first session. This will help ease you in and allow as much time as possible to focus on you.
If you feel that you would like to collaborate your session with another provider or person in your life, simply fill out the following form.
Cancellations and Missed Appointments
You will not be charged a fee for appointments canceled at least 24 hours in advance. Except for emergencies, if you no show for an appointment or cancel with less than 24 hours notice, you will be responsible for paying your full fee. Attendance is a critical part of therapeutic success. If you have frequent cancellations, or abuse the policy (such as canceling just outside of the allotted time window), we will look at identifying and addressing barriers to prioritizing therapy and a new negotiation for attendance.
If you need immediate attention or are in crisis, please call 911 (available 24 hours/day). Please do not leave emails or voicemails if you are in crisis, as I may not hear or see your message for hours or potentially days, and this puts your safety in jeopardy.
Client Contact Between Sessions
Contact between sessions should be limited to scheduling changes only. However, feel free to send me emails or voicemail between sessions if you have something that comes up that you would like to process during session. The material you sent will be discussed during session time. If you feel you would like to schedule extra sessions or move up your appointment time, please feel free to contact me with that request. Phone calls other than scheduling changes will be prorated at my hourly fee. If I am out of town or unavailable for an extended period of time, I will provide you with a backup clinician if you wish.
